Child Visitation Violation
Notice
This sample "Child Visitation Violation Notice," may
be used if you are having problems exercising your
visitation rights. This letter tries to
persuade your former to comply with the visitation
schedule established in your divorce decree, prior
to any contempt proceedings being filed.
The "Original Notice" is required when filing legal
action. You will want to follow the directions on
page two for having the opposing party served
notice. NOTE: This form is required to be
filed when starting a legal action such as a
Contempt of Court, Modification of a Stipulation,
Motion to Establish Joint Legal Custody and
Visitation, etc. When in doubt submit this
form. This is a sample form.
The "Contempt of Court" form above is a form that
may be used to start a contempt proceeding.
Use this form if you have been wrongfully denied
anything set fourth by the court in your stipulation
(example: visitation, child support, telephone
contact, etc.). This is a sample form.
The "Modification" form may be used to ask the Court
to change the current stipulation. In order
for a modification to occur, you must show the court
that extenuating circumstances exist (example: new
laws, changes in existing laws, long term loss of
income, party moving, etc.) This is a sample form.
Petition for Modification
of Physical Placement Provisions
The "Petition for Modification of Physical Placement
Provisions" form may be used to ask the Court to
modify an existing court visitation order to grant
joint physical care. This is a sample form.
Modification of Child
Support Order
The "Modification of Child Support Order" form may
be used to ask the Court to change the current child
support obligation. This is a sample form.
Stipulated Agreement for
Modification Order
This is a sample "Stipulated Agreement for
Modification Order." This form may be used to
jointly modify an existing decree or stipulation.
